Home Assessments
We will visit your home and review how you are currently managing day to day tasks. We provide practical recommendations to improve your safety and independence.

Home Modifications
Our team can suggest home modifications that can enable you to continue to be safe and independent. Home modifications range from simple - such as installation of a grab rail - to more complex - such as installation of an access ramp. We can also provide recommendations to individuals looking at completing new builds or renovations to ensure ease of use and safety.

Equipment Prescription
Our team carry out assessments and recommend appropriate equipment for individuals to improve their functional ability. We also provide training on how best to use equipment to ensure that clients are set up safely.
Our team can also review any funding options available for equipment such as the Geat2GO program or the state wide equipment scheme (SWEP).

Falls Prevention Education
Our team can help older adults take steps to reduce their risk of falls and improve the safety of their environment. This can be achieved by identifying individual risk factors and providing education to change the way in which individuals carry out everyday tasks. We can also make recommendations about home modifications and equipment.

Pain Management
Our team work with both the client and their carers to provide a holistic approach to pain management. Interventions include but are not limited to; education, decreasing pain states, identifying tasks that are meaningful to clients with the aim of enabling participation and problem-solving barriers to engagement.

Cognitive Assessments & Retraining
We can carry out a wide number of cognitive screens and assessments and provide strategies to assist individuals living with any degree of cognitive decline. This can include education and compensatory strategies and the use of technology to maximise participation in day to day tasks.

Functional Assessment & Retraining
We can assess an individual’s current level of function when completing daily tasks.
We then develop recommendations to improve an individual’s level of independence and alleviate any difficulties that are occurring.
Functional assessment and retraining can be completed both in the home and out in the community and are based on an individual's specific goals.
